What Is a Monthly Supervision Report?
Form PROB 8, Monthly Supervision Report , is a legal document completed by offenders on probation to notify the Probation Service about their current residence, employment, and compliance with probation conditions. Supervising offenders in the community is the main purpose of the Probation Service. An offender on probation must be in regular contact with their supervising probation officer and fill out a Supervision Report once a month.

US Supervision Reports must contain the following details:
- Offender's name, date of birth, name of the court, and name of the probation officer;
- Residence information - actual and mailing addresses, telephone numbers, email, names of the individuals living with you, date of the moving and reason for it if you moved during the month;
- Employment details. Identify your employer, name your immediate supervisor, state the position you hold, your gross wages, and normal work hours. If you changed jobs or were terminated, explain why;
- Description of all vehicles you owned or drove;
- Monthly financial statement - net earnings from employment, total monthly cash inflow, and outflow. If you have a post office box or storage space, describe it. If you, your spouse, or dependant have a checking or savings account you regularly use, state the name of the bank, number of the account, and current balance;
- Conditions of supervision. Describe in full detail the particulars of potential arrests, contacts with people having a criminal record, firearms, illegal drugs, travel without permission, and fines. If you have community service work to perform or substance aftercare sessions to attend, check the appropriate boxes and explain why you skipped any of it;
- Offender's signature and the actual date of signing.
