What Is Form CT-W4?

Form CT-W4, Connecticut Employee's Withholding Certificate , issued by Connecticut Department of Revenue Services with the most recent revision dated December 1, 2018 . A fillable Connecticut CT-W4 Form is available for download below.

This is a tax form that is normally completed by an employee before starting a new job. If the Connecticut CT-W4 Form is not filed initially at hiring, an employer may withhold tax by default, as if an employee is single and has no exemptions. By providing your employer with correct information on the Withholding Certificate you can make sure that a proper Connecticut income tax will be taken from your income. An employee has the right to change their withholding options at any time during employment.

Form CT-W4 Instructions

Normally, Form CT-W4 has to be filed every year, or every time a change in your tax situation occurs. Such changes can include: getting more than one job, getting an additional non-payroll source of income or any other income in a form of goods, property, or services. It is very important to fill out Form CT-W4 correctly so you will not be paying too little taxes (under-withheld) and owe the State at the end of the tax year, or you will have too much tax taken (over withheld) and get a return of overpaid tax on your annual State tax return.

It is important to consider your filing status and your expected gross income (all income combined). Your filing status may be as a "Single Person", as a "Head of Household", as a "Married Individual" filing either separately or jointly. Generally the filing status provided on Form CT-W4 is the same used on Federal tax application, but certain exceptions may apply. Depending on your particular situation, the combination of your filing status and your expected gross income will help you select your Withholding Code letter. Selecting letter E means no withholding is necessary. To avoid too little tax withheld letter D can be used. Please carefully read instructions on page 2 of Connecticut Form CT-W4 before filing.

How to Fill Out a CT-W4 Form?

To fill out a Connecticut Form CT-W4 you can use a fillable form or a printable form. For a printed form please use blue or black ink. You will need to provide general information such as your full name with middle initial, your Social Security Number, your full address including ZIP code. Select your Withholding Code.

Here are some guidelines for selecting your Withholding Code:

  • Single, with expected income less than or equal to $5,000 - E; greater than $15,000 - F; significant nonwage income - D;
  • Married Filing Separately, with expected income less than or equal to $12,000 - E; greater than $12,000 - A; with significant nonwage income - D;
  • Married Filing Jointly, with expected combined income less than or equal to $24,000 - E; with spouse employed and our expected combined income greater than $24,000 and less than or equal A to $100,500 - A; spouse not employed and expected combined income greater than $24,000 - C; spouse employed and expected combined income greater than $100,500 - D; etc.

Sign, date, and make a copy of all documents for yourself. Provide your CT-W4 form to your employer.
