What Is Form 4506-T Used For?

The 4506-T Form is used for requesting different types of information. That information can be:

  • Return Transcript. This type of transcript includes most of the line items of a tax return filed with the IRS. A return transcript is only available for certain types of forms, which can include: 1040 Series Forms, Form 1120, Form 1065Form 1120-H, Form 1120-L, and Form 1120-S;
  • Account Transcript. This provides information on the financial status of the account, such as penalty assessments, payments made on the account, and adjustments made by the applicant or the IRS after the return was filed;
  • Record of Account. If a filer is not sure what kind of transcript they should request, it is recommended to order a record of the account, because it provides the most detailed information. It compiles the Return Transcript and the Account Transcript;
  • Verification of Nonfiling. Taxpayers can request verification of nonfiling if they need to get proof from the IRS that they didn't file a return for the year;
  • Form W-2, Form 1099 series, Form 1098 series, Form 5498 series transcripts. If requested, the IRS can provide transcripts from these information returns.

How To Fill Out 4506-T?

Completing a 4506-T Form is a straightforward process. However, an applicant should be aware that if the document is incomplete or contains any invalid information, their request can be rejected.

The application consists of a few lines in which a filer is supposed to fill out the gaps, which includes the following:

  1. Name Here an applicant should enter their name as shown on their return. If the application is filed jointly, then an applicant should enter the name shown first;
  2. Spouse's Name (if applicable);
  3. Social Security Number. Or individual taxpayer identification number (depending on what is shown on the return), or employer identification number (if the request is related to a business return);
  4. Second Social Security Number (if filing jointly with a spouse);
  5. Current Address. Here a taxpayer should designate their address, including apartment, number, city, state, zip code;
  6. Previous Address. Taxpayers use this field to state the previous address shown on their last return if it's different from their current address;
  7. Customer File Number. Completion of this line is optional, however, filers can use this section to enter ten (or less) numeric characters to create a unique custom file number that will appear on the transcript they requested;
  8. Item Requested. A filer must enter the tax form number here and choose the type of transcript they want to request;
  9. Period Requested. Applicants must enter the ending date of the year or period they requested;
  10. Signature. An application should be signed after all the items on the application are filled in;
  11. Title According to 4506-T instructions, if the document is submitted on behalf of a corporation, estate, trust or partnership, a filer should designate their title and attach an authorization document;
  12. Spouse's Signature (if applicable).

Where to Mail Form 4506-T?

After completing a 4506-T tax form a filer must submit it to the IRS. The address depends on the state where an applicant filed an individual return and lived in. If an applicant requested an individual transcript (Form 1040 series, Form W-2, and Form 1099), they should mail or fax the application to:
