PS Form 3533, Application for Refund of Fees, Products and Withdrawal - also known as the USPS Form 3533 - is a form used for requesting a refund for postage and fees. The latest available version of the form was released by the United States Postal Service (USPS) on August 1, 2008 .
A printable PS Form 3533 can be downloaded below for reference. Every USPS refund form has a 20-digit unique barcode on the top right-hand side, making it impossible to use a standard print-out for all transactions. Each individual customer refund or indemnification transaction is tracked and processed on a separate form.
As of today, no PS Form 3533 fillable copies are available online, but you can order a hardcopy of the form through a Material Distribution Center. Use touch-tone order entry (TTOE) by calling 1-800-273-1509. If you are not registered to use TTOE, call 1-800-332-0317, option 1, extension 2925.
Take your package and the PS 3533 to your local post office and submit them to a postal employee. All refunds need some proof of purchase. It can be an envelope or a wrapper with names and addresses, tracking number, purchase receipt or photo ID. Postmaster will calculate everything to verify the refund amount. The fee for each processed refund is calculated at 10 percent. If the 10 percent fee is more than $350, you pay a flat fee of $35 an hour. After verification of the refund amount, a post office will issue a no-fee money order (for refund less than $500) or send a PS 3533 with Account Identifier Code (AIC) 280 and supporting documents to a disbursement center for refund payment.
Effective October 2, 2017, the USPS added the option to file the application digitally online request. A mailer can submit the online refund application at the USPS website. While the refund request is being processed, the mailer has to be on hold with evidence of services purchased and may be required to upload them and provide the mailing date. In case of a negative response, an online refund request must be submitted within 30 days.
