What Is PS Form 3189?

PS Form 3189, Request for Temporary Schedule Change for Personal Convenience , also known as USPS Form 3189, USPS Temporary Change of Schedule form, or USPS Change of Schedule form is a Postal Service worksheet. This worksheet is used for requesting the United States Postal Office (USPS) to temporarily change regular timetable within a specified period (days, weeks, months). It is used by a USPS employee for their personal convenience. By submitting a properly filled in (and approved) petition to the supervisor, an individual agrees to forfeit any out-of-schedule premium, to which they would otherwise be entitled to. The appropriate union official must accept the temporary change before it is submitted for approval.

PS Form 3189 Instructions

Follow these steps to fill out and submit the form:

  1. Enter your printed name (first and last, as listed in postal records).

  2. Specify the period (days, weeks, months for a temporary change.

    • "From Date" - start date for the change;
    • "Through Date" - the last date for the change. If it is for the entire week, the last date would be Friday.
  3. "From Regular Schedule." The first timetable listed in TACS (Time and Attendance Collection System) for the first workday of the employee's schedule. "Change Schedule To:" - first day of the newly proposed timetable for the pay week bearing in mind the basic clock rings (BT, OL, IL, ET). "Primary" stands for the main (principal) timetable.

  4. Individual days in both "From Regular Schedule" and "Change Schedule To" should be filled in only if needed. They are used when an employee has daily varied timetables or when one or more days have been nominated for change.

  5. From "Scheduled Day(s) Off" the employee's non-scheduled days as stated on their current job assignment. To "Scheduled Day(s) Off" the timetabled day(s) off being applied for.

  6. "Employee's Signature." Self-explanatory/

  7. "Employee's ID (EIN)." The employee's full eight-digit EIN.

  8. "Date Signed." Self-explanatory.

  9. "Pay Location." Official pay location.

  10. "Steward's Signature." A legible signature of the union official condoning and agreeing with the employee's application.

  11. "Date Signed." Self-explanatory.

  12. "Approved." Self-explanatory.

  13. "Disapproved." The supervisor must enter a reason in the space provided.

  14. "Supervisor's Signature." A legible supervisor's signature.
