What Is PS Form 1188?
PS Form 1188, Cancellation of Organization Dues From Payroll Withholdings , is a form used by employees of the United States Postal Service (USPS) to cancel labor organization dues from the employee's pay. A labor organization protects and promotes the interests of its members in regard to working conditions, wages, and other work-related issues. In return, the members share dues withdrawal to finance the organization.
This form, also known as USPS Form 1188, must be submitted for discontinuing payment withdrawals only in accordance with the terms of the authorization form that the employee signs to initiate the dues withholding. The cancellation form should be submitted at least 10 days prior to the payment withdrawal anniversary date and not earlier than 20 days before that day. The form is effective immediately after it is approved.

How To Fill Out PS Form 1188?
Instructions for completing the PS Form 1188 are provided below.
Part A of the Form should be completed by the employee.
- Block 1, Employee Name. Enter your full name;
- Block 2, Social Security Number. Provide your SSN;
- Block 3, Post Office Name and State. Enter the name and the state of the post office you work at;
- Block 4, Post Office Finance Number. Enter the finance number of the office you work at;
- Block 5, Name of Organization and Code. Enter the organization name and code. The list of organizations and codes is provided on the reverse side of PS Form 1188;
- Block 6, Employee's Dues Deduction Anniversary Date. Fill in the date you initiated the dues withholding;
- Block 7. Certify that you sent the original of the cancellation form and three copies to the organization's national office as notice of your intention to discontinue payment withdrawals. Enter the date of sending the copies. This postal operation must be executed via certified mail;
- Sign and date Part A;
- Part B is completed by the Employing office. The date the form was received is entered in this part and the form is forwarded to the processing center;
- Part C is completed by the processing center. This part includes the date the form was received, organization code, effective pay period, date processed and the signature of the verifying person;
- If the form was not processed due to any reason, it is returned to the employing office, where the installation head informs the employee about the return. The reason is mentioned at the bottom of the page.
Where to Send PS Form 1188?
The completed PS Form 1188 should be submitted to the installation head of the employment office. Also, as it was mentioned in Part A block 7, the original of the cancellation and its copies should be sent to the organization's national office. Then the original of the form and two copies are forwarded to the HRSSC for processing.
