What Is PS Form 3849 Used For?

The purpose of the form is to provide an improved and efficient service to USPS customers. This form is used by postal service employees to inform customers about an unsuccessful delivery attempt. This allows mailers to let the customer know when the postal service will try to deliver the mailpiece again and in which postal office the package is located.

PS Form 3849 Instructions

The first time a USPS employee attempts to deliver a mailpiece but fails to do so because the customer is not available to receive the item, they are required to fill out a PS Form 3849 and leave the notification for the customer. The form must include information about the second time the employee will try to deliver the mail (which should be the following working day) and the USPS office where it will be held for the time being.

If the second attempt fails as well, the employee will leave a second copy of the PS Form 3849 to inform the customer of this attempt and when the article will be held at the office for a pick-up. The mailpiece will be stored at the USPS office until the customer claims it. However, if the customer does not pick it up within 15 days from the first attempted delivery, the item will be returned to the sender.

In order to get their mail at the post office, the customer must bring their PS 3849 and present a proper ID. Every customer who receives this form at their address has several options to schedule a redelivery. For example, a scannable QR code can be used with smart devices. Or, instead of calling the USPS, redeliveries can also be managed through a feature called Informed Delivery, which allows the customer to digitally preview and manage their incoming mail from a computer or mobile device. Also, the recipient can authorize another person to sign for their mail on their behalf.
