What Is Form I-134?
Form I-134, Affidavit of Support , is a statement completed by U.S. citizens and lawful permanent residents (green card holders) to show their intent to support non-U.S. citizens financially during their visit to the U.S. This document is often used for visitor visas and tourist visas where the beneficiary's financial situation does not avoid the risk of being found inadmissible to the U.S. as a likely public charge. The U.S. government needs to see the foreign visitor will not become a liability, will not need to find a job, or seek public benefits - however, approval of the affidavit does not guarantee the issuance of the visa.

How to Fill Out Form I-134?
Provide the following information in the I-134 Form:
- Provide your personal information - full name, other names you used, date, and place of birth. Indicate your mailing and physical addresses - if you change them, notify the USCIS of your new address within ten days of moving. If you have an alien registration number (A-number), social security number, and USCIS online account number, record them. Check the appropriate box to show the way you obtained U.S. citizenship unless you were born in the U.S. - you may be a citizen through naturalization, through parents, or marriage. If you are a lawful permanent resident or lawfully admitted nonimmigrant, enter your A-number or Arrival-Departure record number. State your age and the time you have resided in the U.S.
- Write down personal details of the beneficiary, including their gender, country of citizenship or nationality, and marital status. Describe your relationship with the visitor. Add the beneficiary's physical address. Name the beneficiary's spouse (who will accompany or follow the beneficiary) and children.
- Provide your employment details - name and address of the employer. Indicate your annual income, the balance of your savings and checking accounts, the value of your property, stocks, and bonds. If you have life insurance or own real estate, state its value as well. Name individuals who depend upon you for support and explain their relationship to you.
- Confirm you have read and understood the form, provide your contact information, and certify the statements in the affidavit are true and correct. Sign and date the form. Signing this affidavit, you show you are prepared to receive, maintain, and support the individual named in the form.
- If you were helped by the interpreter or preparer when filling out the affidavit, they also need to add their full names, mailing addresses, contact details, and signatures to the form.
- If you need extra space to submit more information, you may use Part 7 of the form. Make copies of the last page of the form and attach them to the affidavit. In case you wish to learn more about this affidavit, you may consult with the official Form I-134 Instructions, also issued by the USCIS.
Where to File Form I-134?
You need to send a properly signed Affidavit of Support to the foreign visitor it is prepared for. Then, this individual submits Form I-134 to their U.S. consulate with a visa request. The USCIS does not charge any filing fee for I-134 Form. The sponsor must also provide the visitor with evidence to show sufficient income and financial resources.
Form I-134 supporting documents include bank statements, employer letter that states the sponsor's position and salary, copy of the last income tax return, list of bonds, etc. Additionally, you need to present a copy of the document that shows your citizenship or immigration status - a copy of a U.S. passport, green card, or naturalization certificate. Without these documents, your affidavit may be rejected or denied.
How to Withdraw Affidavit of Support (I-134)?
If you change your mind and no longer can or want to support your foreign nationalfamily member or friend, you may withdraw Form I-134. Whether the USCIS approved your affidavit or did not, you may easily withdraw the support. Write the USCIS office that is currently reviewing your affidavit. Request the withdrawal, provide your full name, date of birth, personal details of the beneficiary, and the identifying number for your affidavit that you can find on the receipt notice - Form I-797, Notice of Action. If you wish, you may disclose the basis for withdrawal - if you do not, it will not affect any future petitions or affidavits you will file in the future. Once the request for the cancellation of an affidavit is received, the USCIS will review it and mail you a confirmation of the withdrawal.
