The DD Form 1173-1 is the United States Uniformed Services Identification and Privilege Card for National Guard and Reserve service members. This ID Card is issued by the Department of Defense (DoD) to dependents of Reserve members not on Active Duty or in receipt of retired pay, former members not in receipt of retired pay and other eligible individuals described by DoD policy.
The DD 1173-1 serves as proof that its holder has been pre-enrolled in the Defense Eligibility Enrollment System (DEERS) and does not authorize eligibility for any medical or insurance benefits.
The card will assist family members of the National Guard and Reserve component members in gaining access to their privileges. The DD Form 1173-1, when submitted with a completed DD Form 1172-2 Application for Identification Card/DEERS Enrollment, will allow family members to receive the DD Form 1173, which grants access to applicable medical, commissary and exchange benefits.
