DA Form 2402, Maintenance Tag , is the updated version of the previously-used DA Form 2402, Exchange Tag . The current edition was adopted by the U.S. Department of the Army (DA) on December 1, 1985 . The form is unclassified and approved for public release but is not available online. The 1985 version of the form is provided below for reference.
The form is intended to serve as an identification tag. It is used as a receipt for Test, Measurement, and Diagnostic Equipment (TMDE) items needing calibration, to identify items held for warranty claims and Electronic Inventory Record (EIR) exhibits, etc. DA Form 2402 is often confused with the Department of Defense-issued DD Form 2402, Civil Aircraft Hold Harmless Agreement, which is a form used to act as an agreement between the U.S. Army and an individual or company wishing to use military airfields.

A separate copy of the form is used for each item separate item described. DA Form 2402 is completed in four copies. Not all copies may be needed depending on the item and the number of repairs required.
When an item or part is installed or disposed of, DA Form 2402 that applies to it is destroyed with all copies used as receipts. If the form identifies a warranty claim or Form SF-368 exhibit, the tag stays with the equipment until that equipment is no longer needed for that claim or exhibit.
