CBP Form 7512, Transportation Entry and Manifest of Goods Subject to CBP Inspection and Permit , is a form used for sufficiently describing the cargo to identify it for the Customs and Border Protection (CBP). This document is a compilation of information about the goods carried on a means of transport. It generally shows the goods' consigner and consignee, lists the merchandise details and characteristics such as origin, value, number, and destination. This form is necessary for brokers and carriers who transport merchandise in bond (an export or import shipment that has not yet been cleared by the customs) from a port of importation to another CBP port.
The latest version of the form was released on January 1, 2018 , with all previous editions obsolete. A CBP 7512 fillable form is available for download and digital filing below.
CBP Form 7512A, Continuation Sheet, is a form used as a supplement to CBP Form 7512 and contains information that did not fit in the first form. It includes the description and quantity of merchandise, the kind and the number of packages, its gross weight in pounds, and value (in accordance with the shipping papers).
Consult the CBP officer to know the appropriate number of copies required for entry and/or manifest purposes. Be ready to provide necessary information on a proper bond to the place of shipment, extra copies with a stamp, or notation relating to their intended use.
Make sure the kind of entry is clearly shown in the block in the upper right-hand corner on the first page of the form, whether it is used as an entry or withdrawal. Verify that the merchandise has been delivered in good condition as noted on the original lading.
The private parties may print the form if the printed version conforms to the official version in wording arrangement, size, color, and quality of the paper. All copies may be prepared simultaneously by carbon process unless more than one vehicle or vessel is used. In that case, prepare a separate manifest for each such vehicle or vessel.
