SBA Form 3301, Nomination Form for Small Business Person of the Year is a form used to nominate a person, their business, and their partners for the Small Business Person of the year award. The form can be used for self-nomination or to nominate a candidate for the award.
The latest version of the form was released by the Small Business Administration (SBA) on September 1, 2017 , with all previous editions obsolete. An up-to-date SBA Form 3301 fillable version is available for digital filing and download below or can be found through the SBA Forms website.
Every nominee for each SBA award must complete the SBA Form 3300, Award Nominee Background Form. The form requires general information on the nominee and history of any arrests and convictions they may have had. If a team is nominated for the award, each member of the team has to file a separate SBA 3300.

The information provided on the SBA Nomination Form 3301 is used to determine the nominee's eligibility for a particular small business award, to identify any actual or apparent conflict of interest and to make eventual award determinations. Providing incorrect information will lead to disqualification and might result in penalties. The completed SBA Form 3301 is submitted as part of the nomination package along with the SBA Form 3300, Award Nominee Background Form completed by each co-nominee.
Any legal owner of small business within the United States and can be nominated for SBA awards either individually or along with a team of employees or partners. Business owners that received an award from the SBA in the last three years cannot be nominated.
All award application packages must contain a filled-out copy of the Nomination Form for Small Business Person of the Year and an Award Nominee Background Form and must be submitted to a local SBA office before the annually-set deadline. Winners of the District office awards will be nominated for State-level awards and State-level award winners will be nominated for the National level awards.
