SBA Form 1031, Portfolio Financing Report is a form completed by small business investment companies (SBICs) to provide the Small Business Administration (SBA) with financial information used for compiling statistical data. All SBA-licensed SBICs should submit their Portfolio Financing Report on the SBA 1031 within 30 days of the closing date for each Financing of a Small Business.

The actual report must be filed electronically and submitted through the SBIC-Web system. using an SBA-approved user account. Request an account by sending an application to sbicwebsupport@sba.gov.
The top two lines of the form require the name of the licensee and their license number.
Part A is for small business concern data.
Part B is for providing pre-financing information.
Part C is for providing financing information.
Part E is for the verification of transmission and for the date of transmission.
