What Is DA Form 4591-1?
DA Form 4591-1, Status of the Retention Data Worksheets , is a form that summarizes the assigned strength of a single unit and provides the statuses of all individual DA Form 4591, Retention Data Worksheet, copies used by the soldiers of that unit. The assigned strength of a unit is the total number of enlisted soldiers not serving under the indefinite reenlistment program.
The DA 459-1 Form should be used in connection with a locally produced or automated unit personnel accountability roster and is filed in accordance with the Army Regulation 601-280, Army Retention Program.
The latest version of the form - sometimes incorrectly referred to as DD Form 4591-1 - was released by the U.S. Department of the Army (DA) on March 1, 2016 . DA Form 4591-1 Instructions
The DA 4591-1 is used together with DA Form 4591, Retention Data Worksheet, and DA Form 4591-2, Record of Inspections Conducted. All three forms are maintained by the company commander and company retention non-commissioned officer (NCO).
The worksheets are kept together in an accordion file, a three-ring binder, or in a filing cabinet. The file consists of three sections with the three forms kept between separate section dividers.
- Section I ("Status of DA Form 4591-1") is where the company commander or the retention NCO will maintain copies of DA Form 4591-1.
- Section II ("Inspection Record") is the divider behind which the retention NCO will keep copies of DA Form 4591-2, Record of Inspections Conducted, along with a copy of all inspections reports and staff assisted visit (SAV) findings, performed during the last year.
- Section III ("Suspense File") consists of a "Suspense Divider," seven yearly dividers, and a single set of twelve monthly dividers used for storing soldier's DA Form 4591 copies.
