Maryland Change of Address Instructions

The instructions for filling in Form VR-154 are the following:

1. Section A, All Applicants.

   • The applicant should enter their driver's license number. If the applicant does not have a Maryland driver's license, they should print their Soundex number;
   • The applicant has to enter their date of birth, full name, old and new resident addresses, and title number before signing and dating the form.

2. Section B, Voter Registration.

   • If the applicant doesn't want to update their voter registration address, they should check the applicable box on Item 1;
   • In the case of new registration to vote, the applicant should complete Items 2 - 14. The applicant has to answer the questions about their age and citizenship, indicate their gender, daytime phone, and Social security number;
   • If the applicant wants to change their party affiliation, they have to complete Items 3, 9, 13, and 14. Check the appropriate reason for applying and choose a political party to register with;
   • Sign the form, enter the date, and address on the last registration.

3. Sections C and D are informative and contain information for all applicants and regarding voter registration.
