Form MC-031, Attached Declaration , is a document approved for optional use by the California Judicial Council. The form was revised on July 1, 2005. A fillable Form MC-031 is available for download below.
MC-031 Form is supposed to be filled out by an applicant at their discretion, as a supplement to Form MC-030, Declaration.
The purpose of the form is to give an applicant more space for writing so that they will be able to submit as much information as necessary to the court. An applicant can be any person that has dealings with a lawsuit in California and can represent any party: the defendant, the plaintiff, the attorney, the petitioner, etc.
As mentioned earlier, Form MC-031 is used as an addition to Form MC-030. When an applicant runs out of space on Form MC-030, Declaration, they are allowed to attach Form MC-031 in order to provide any extra essential data. The particular use of the form depends on the declaration it accompanies. The form becomes valid in the court when it is attached to any judicial declaration.
The Declaration Form MC-031 must be submitted by an applicant to the court in the State of California, where the proceedings of a case take place. After it is considered by a judge, it becomes a part of the case.
You then have to sign the form and specify the party of the case you represent. The signature certifies that the data in the form is true and correct. The form can never be submitted by itself, it must always be attached to another form or court paper (only then it can be filed in court).
Despite the fact that Form MC-031 is not used as an independent form, its significance is hard to underestimate. With its help, a party can present any amount of data to the court during the case to help validate their position.
