Form MC-030, Declaration , is a form used by any participant of legal proceedings (an attorney, respondent, or defendant) to provide the court with any information they deem relevant to a case. It may be attached to another court document. Declarants state in their own words what they know about the cause of action to be true. This written statement is sworn to be true and accurate under the penalty of perjury by any individual with direct knowledge about the issues in a court case. Anything the judge has to know to make a decision at the hearing should be included in a declaration.

Form MC-030 instructions are as follows:
Take time to organize your ideas before you complete the document. Stick to the main points and do not add anything unrelated to the case. Describe specific things and give details about when and where incidents took place;
Ask other people who have directly witnessed the important events to write declarations in order to explain what they saw or heard. You may ask friends, neighbors, relatives, doctors, law enforcement officers, and clergy members to provide specific information. However, do not forget the judge gives more weight to a professional or neutral individual than someone who obviously supports only one side of the case;
Attach written proof to the declaration if it helps prove what the declaration says. Provide bills, medical records, police records, insurance records, financial statements, and other papers you would like the court to see. Refer to these exhibits in the declaration.
