Form DS-86, Statement of Non-receipt of a U.S. Passport , is used when you have completed a form requesting a new or updated passport and it has not arrived in the mail within 10 days of it being sent out. It is very important to complete Form DS-86 when you believe that your passport has gone missing or stolen since identity thieves could use it.

Before completing the DS-86 Form, you should carefully read through the notice at the top of the page and the warning and Privacy Act Statement at the bottom of the form to make sure it is completed correctly. Should the passport arrive after you have completed this form, you will need to reapply for a passport, since the department will have invalidated the passport and it will not be a valid form of identification.
Once you have completed the form, you will need to contact the Bureau of Consular Affairs and speak to or email a representative about how to submit the form. The form also lists the times on Monday through Saturday that you will be able to call and speak to a representative. Currently, there is no way to submit it through their site without contacting a representative first.
There is only one section that you will need to complete which will ask what product you have not received (either a "U.S. Passport Book and Card," "Book Only," or "Card Only"), your personal information, what steps you have taken to locate the passport if the documents you submitted for your original passport were returned to you, and a section where you can explain in more detail what documents you submitted that had not yet been returned to you.
This final section to complete is a signature section that has an additional signature space provided for guardians to complete if necessary.
