Form CG-7421B Instructions

Provide the following details in the CG-7421B Form:

• Enter your full name, military rank, and employee identification number. Record the names of your department and unit. Add your phone number and email address;
• Choose a role for your access. You may select between command, administration, reserve only administration, mobilization, training, security, and various/others. Different roles allow different levels of access - for instance, view-only access. You are not allowed to access the system for purposes beyond the scope of your authorization;
• Acknowledge the authorization to access the Direct Access system and sign the form. Since the form has to be filled out online, you may click on the field "user click here" to email the form to your authorizing official.

The rest of the form must be completed by the authorizing official:

• Certify that the authorized access is based on an official need;
• State your employee identification number, rank, title, and telephone number;
• Sign the form and send the document directly to the Pay and Personnel Center via email.

Form CG-2002, CG Direct Access System Assignment Data Worksheet, is a related document you need to complete to update assignment data, for example, if you want to request a transfer.
