DA Form IMT WF1, Hurt Feelings Report , is a form issued by the U.S. Department of the Army (DA). It was originally released on April 1, 1989, along with Field Manual 22-102 "Wall-to-Wall Counseling" (FM 22-102) as a joke.
The latest version of the report was released on April 1, 2009 , according to which the edition of April 1989 is now "obselete," intentionally spelled incorrectly. A fillable version of the Hurt Feelings Report is available for download below.
A Hurt Feelings Report consists of several parts. It starts with a title and a reference to Field Manual 22-102, which should be used along with this report. The next section is Data Required by the Privacy Act of 1974. This part of DA Form IMT WF1 consists of several types of data:
The purpose of the form is to assist "whiners" in documenting their hurt feelings and to provide leaders with a list of soldiers who require additional counseling, non-commissioned officer (NCO) leadership, and extra duty.

In the "Narrative" part of the Hurt Feelings Report, the filer is offered to tell in their own words how their feelings were hurt. The last part of the Hurt Feelings Report is "Authentication." This segment authenticates the names and signatures of those who were involved in the incident: "real man/woman" and "whiner." At the end of the form, the U.S. Army claims to take hurt feelings seriously and promptly dispatch a "hugger" to anybody that needs one, as well as do other things to make the filer feel better.
The Army Hurt Feelings Report and Field Manual 22-102 "Wall-to-Wall Counseling" was released to commemorate April Fool's Day in 1989. The intentional use of sarcasm and spelling mistakes in the form doesn't have any other purpose except for humor.
