How to Fill Out DA Form 1574-2?

Additional information and filing guidelines and can be found in the Army Regulation 15-6, Procedures for Administrative Investigations and Boards of Officers. DA Form 1574-2 instructions are as follows.

1. Section I contains the information about the appointing authority and the date of appointment.
2. Section II describes the sessions held by the BO. The required information includes the time of the session, the place of the session, the participating officers and members absent at the meeting.
3. Section III includes a checklist of proceedings. The Recorder marks the necessary boxes according to the facts of the investigation. Part A is filled out for all cases. Part B may be completed only for cases with a designated respondent.
4. Section IV summarizes the findings of the investigation. Each paragraph must relate to one evidence-based conclusion and answer one question raised in the memorandum by the appointing authority.
5. The BO gives their recommendations in light of the findings in Section V. All the members must provide their signatures in Section VI to verify that the report is accurate and complete.
6. Section VII is completed by the appointing authority. The appointing authority may return incomplete, ambiguous, or erroneous reports to the BO for revisions.
