Follow these steps to fill out CDL Self Certification Form:

  1. State your name and the number of the driver's license issued in Ohio.
  2. Select the category that applies to your situation. Your commercial driving may involve interstate or intrastate driving with or without the need to comply with medical examination requirements. The second page of the form contains sufficient descriptions of all categories and examples of vehicles - make sure you choose the right one. Note that if you travel between states, you will have to attach a medical card to the certification. This document will confirm you are physically prepared to drive a commercial vehicle.
  3. Sign and date the form.
  4. Check the box to certify you have enclosed a medical examiner's certificate if necessary.

Once the certification is filled out, send it to the Ohio BMV, CDL/In-state Violations Unit, PO Box 16784, Columbus, Ohio 43216-6784, or submit the documentation to a local regional reinstatement center. When you receive the license in the mail, you must keep it with the medical waiver in case you do not have to meet the medical requirements prescribed by the state.
