PS Form 1500, Application for Listing and/or Prohibitory Order is a form used to stop the delivery of unsolicited mail. Also known as USPS Prohibition Order, this form was originally intended to stop the delivery of pornographic materials, but it can be used to stop the delivery of any kind of offensive mail.

The opened unsolicited mailpiece should be attached to the completed PS Form 1500, and the materials should be delivered to the Postal Service. The USPS will issue an order to the mail sender that demands them to stop sending offensive correspondence. Further attempts to send junk mail to the prohibited address will be considered a crime.
If you do not want to get offensive mail anymore, the most effective way to stop it is to obtain a prohibitory order against the sender. PS Form 1500 serves this purpose.
You will receive a reply within 15 days. The reply will contain a confirmation letter that says they have stopped the junk mailer from sending letters to your address. The Prohibitory Order against the sender lasts 5 years. It will take effect 30 days after the sender receives the order from the post office. The Prohibitory Order makes the sender refrain from correspondence to your address and deletes your address from all their mailing lists controlled by them directly or by their agents or assigns.
If after obtaining the Prohibitory Order you receive junk mail from the same sender again, you should place the date on this junk mailpiece and send it to the United States Postal Service with the attached copy of the Prohibitory Order.
