Form VSA 66 Instructions
Provide the following information in Form VSA 66:
- Application type (transfer of lien or supplemental lien).
- Owner details (full name, phone number, DMV customer number, federal employer identification number, or social security number, and address). Indicate whether the vehicle owner is on active military duty.
- Vehicle description (identification number, title number, plate number and type, year, make, model, body type, and weight).
- Information about the current lien (lienholder's name, code, and mailing address, and the date of the lien).
- New lienholder details (if you transfer the lien).
- Supplemental lien details (if you need to add a supplemental lien).
- Confirmation of the statements in the application, owner's signature, and the date of signing. The form must be signed and dated by the current and new lienholders as well.
Once you complete the application, bring it to any DMV customer service center or mail it to the Virginia DMV, PO Box 27412, Richmond, Virginia 23269-0001.
