What Is Form DS-11?
Form DS-11, Application for a U.S. Passport , is a legal document used by U.S. citizens and nationals when applying for their first passport. In addition to this completed application, it is required to submit a copy of an identification document - birth certificate, social security card, or driver's license. This form was released by the U.S. Department of State (DS). The latest version of the form was issued on June 1, 2016 , with all previous editions obsolete.

How to Fill Out a U.S. Passport Application?
Form DS-11 instructions are as follows:
- Select the documents you are applying for - passport book, passport card, or both.
- Write down your personal details - full name, date, and place of birth, sex, social security number. Provide your contact information - email, phone number, mailing address. List all other names you have used.
- Indicate the personal details of your parents, including their full names, dates, and places of birth, sexes, citizenship.
- If you have ever been married, state the full name, date, and place of birth of your most recent spouse. State if the spouse is a U.S. citizen and records the date of marriage. If you have ever been widowed or divorced, check the appropriate box and write down the date of widowhood or divorce.
- Provide an additional contact phone number, list your occupation, employer, or school.
- Enter your personal details - height, hair color, eye color.
- Describe your travel plans - the dates of departure and return, list countries to be visited.
- State your permanent address and provide the emergency contact information.
- If you have ever applied for a passport book or passport card, write down your name as printed on it, number, date of issue, and it's status.
- Sign the form in the presence of the designated acceptance agent.
Where to Send U.S. Passport Application?
Mailing in your U.S. Passport Application is not allowed: you can only submit the papers in person. There are two ways to submit Form DS-11, both depend on where you're applying from:

Inside the U.S. Submit the application along with citizenship evidence, copy of the ID, photo, and fees at a passport acceptance facility - post office, clerk of court, public library, or another state, county, township, or municipal government office that is allowed to accept the DS-11 Application Form.

Outside the U.S. Contact the local U.S. embassy or consulate to find out how to submit an Application for U.S. Passport, since processes are different.
How to Track U.S. Passport Application Status?
You may check the status of your U.S. Passport Application Form online or by phone. Your last name, date of birth, and the last four digits of your social security number are required to see the application's status. After you have applied for the passport, it takes 7-10 business days for your application to be trackable online via the Online Passport Status System.
