State Pension Claim Form Instructions

The following information is required on the State Pension Claim Form:

1. Your National Insurance Number, full name, address, phone number, and date of birth. You will also need to include your original birth certificate or a copy that has been certified by an authorized professional.
2. You will be asked about any addresses you maintain in the United Kingdom, the length of time you have spent in the United Kingdom (including any holiday time), and if you have ever served in a branch of the armed services. If you are or were employed by an entity located in the United Kingdom, you will need to include the name and information about the employer and the dates you worked for them. You will also be asked about previous tax contributions you made.
3. Your current marital status and general information about your current marital spouse or civil partner.
4. Additional information about your birth records, marriage status (or record of divorce or widowhood if applicable), or death of a spouse (and the location where this occurred).
5. The date you wish to begin claiming your benefits (you must choose a date to begin receiving payments) and how often you wish to be paid.
6. If you or your spouse or partner received any additional benefits through the state or child benefits.
7. Details about your nationality and the countries you lived or worked in outside of the United Kingdom and the length of time.
8. Bank account information.
