What Is PS Form 991?
PS Form 991, USPS Application for Promotion and Assignment - also known as the USPS Form 991 - is used to apply for any Executive Administrative Schedule (EAS) position in the United States Postal Services (USPS) including the position of postmaster and initial-level supervisor. It can be used by bargaining employees to show their qualifications to apply for a promotion.
The form was revised on June 1, 2014 . An up-to-date PS Form 991 fillable version is available for download below. The form should be sent to the office that announced the job opening at the address indicated in the vacancy description.
PS Form 991 Instructions
This form is filled out by the applicant's information, i.e. education, work experience, capabilities and skills that go with the requirements of a certain position that the review committee and selecting officials seek for.
Here are the changes that you may notice in the 2014 PS 991 Form:
- The edited Instructions page is at the beginning of the form;
- The Notes to Applicant page is edited for bargaining vacancies, the Privacy Act Statement has been updated and the Computer Matching System has been deleted;
- The Employee Identification Number is required instead of the Social Security number;
- The number of pages has been edited;
- Two pages that are specifically related to non-bargaining vacancies have been deleted.

How to Fill Out PS Form 991?
The form requires the following information:
- Applicant Information. Enter your personal information, such as your name, title of your present position, mailing address, home phone number, employee ID number;
- Enter information about the vacant position: vacancy announcement number, closing date, name of vacancy office, position you are applying for;
- Education/Training. Enter your educational background: diplomas, educational certificates and training courses that qualify you for this position;
- Your previous postal positions starting with the current position (in reverse chronological order);
- Specify any non-postal positions you held;
- Provide any of your temporary positions of 30 or more consecutive days, in reverse chronological order;
- Statement of Qualifications. Explain your qualifications devoting one page of the form for each requirement.
