MVT-2 MH Instructions

To complete an MVT-2 MH Form you will need to provide the following information:

1. General information about the applicant, such as their name, address, and phone number.
2. The identification information for the tiny home, the purchase date, the state where the unit was built, and if it is new or used.
• If the home is used, provide the name of the seller and their address.
• If available, include the Maine license number or dealer plate (if purchased in-state).
3. If you require financial assistance when purchasing the tiny home, you will need to include the lienholder's information and address.
4. A Maine dealer's representative will be required to certify the information above is accurate.
5. Complete the final signature section for the owner and parent or legal guardian if the signee is under the age of eighteen.
