AF Form 428, Request For Overtime Holiday Premium Pay and Compensatory Time is a document used to track time, overtime, holiday premium pay, and compensatory time for management and manpower decision support and for payroll/financial purposes.
The latest version of the form was released by the Air Force (AF) on March 13, 2019 , with all previous editions obsolete. AF Form 428 fillable version is available for download below or can be found through the Air Force e-Publishing website.

This instruction prescribes rules for handling overtime; and procedures for approving recruitment and relocation bonuses, retention allowances, and supervisory differentials. It applies to all commanders, civilian personnel officers, staff officers, and supervisors of U.S. civilian employees.
The form can be used for:
The employees are entitled to the same amount of compensatory time off as the number of overtime hours worked, i.e., 1 hour of compensatory time off is granted for each hour of overtime worked;
All working additional hours must be officially ordered and approved in advance by the supervisor using the AF 428. Supervisors have to obtain approval from their overtime authorizing official before ordering overtime. Commanders must continually ensure that overtime work occurs only pursuant to mission needs;
Employees may not receive overtime pay for work which a supervisor did not properly authorize and approve (except employees who agreed to work overtime in accordance with the Fair Labor Standards Act provisions). Employees must provide their supervisor with a copy of their travel settlement voucher overtime is based upon;
Approving officials and supervisors must ensure that justifications on the AF 428 include details requiring overtime to allow an approving official to evaluate and determine the necessity of the overtime work to be performed. Justifications must describe work to be performed, why overtime work is required, and/or what caused the temporary surge in work performed. Overtime must be held to an absolute minimum by making every effort to accomplish work within the normal 40-hour workweek. Only overtime determined to be absolutely necessary to carry out an assigned mission on a timely basis is to be considered for approval;
Supervisors must provide a copy of the approved form to time and attendance approving officials and timekeepers for audit purposes;
There are special requirements for the assignment of overtime when the requirement conflicts with an employee's religious obligations or practices. Supervisors should consider changing tours of duty involving the assignment of overtime thus permitting the employee to perform the overtime on another day. Another option is to excuse the employee from the assignment.
Employees are entitled to Law Enforcement Availability Pay (LEAP) once they complete both the Criminal Investigator Training Program and AFOSI-specific training at the Federal Law Enforcement Training Center, all of which takes 18 weeks. If an employee's training exempts them from this training, the eligibility for LEAP is automatic.
