How to Fill Out an Army Road Test Score Sheet?

DA Form 6125 instructions are as follows:

1. Specify the date the road test takes place . Identify the driver by their full name and enter your own name. State the identification number assigned to the operator by the Department of Defense or the Global Combat Support System identification number and the route you will take. Once the test is over, record the score on the top of the first page of the form.

2. When the license examiner observes the behavior of the driver on the road, they should be ready to find a particular maneuver in the form and be prepared to complete the paperwork while the vehicle is moving instead of filling out the documentation later . As soon as the driver is able to listen without getting distracted, the examiner has to provide them with directions for the next action they have to carry out successfully.

3. Note how the driver approaches different road situations such as dealing with the traffic check or keeping sufficient distance from the car in front of them . Analyze the driver's ability to handle straight sections in urban and rural areas, intersections, and lane changes. Pay attention to the way they turn the vehicle and feel aware of the car's position, speed of the vehicle in relation to the speed limit and other cars around, and possible obstructions in their way. Evaluate how the driver is stopping and starting the vehicle, how they are driving upgrade and downgrade, and whether the car turns are smooth. The form allows the examiner to record both left and right turns depending on the route chosen for the test - you may also enter the locations of the turns keeping track of all of them.

4. Assess the general behavior of the vehicle operator - how well they are able to approach various systems and mechanisms while they are at the wheel, how they are following all the signs and signals, and whether their actions led to you moving abruptly in the car. Make sure you properly indicate all the errors the driver has committed - instead of checking the box that certifies they made no mistakes, elaborate on the weak points of their driving in the notes section. You may also use this field to write down your remarks that are not in any way related to any of the scoring categories and share unexpected events that took place during the test.

5. In case the driver made mistakes, the examiner needs to review those shortcomings and annotate the score sheet . After the evaluation of the driver is over, the copy of the form will remain in the file under the name of a particular operator alongside other documents that describe their performance.
