CBP Form 7512A, Transportation Entry and Manifest of Goods Subject to CBP Inspection and Permit - Continuation Sheet , is supplementary to CBP Form 7512 and is used to provide additional description of the cargo to help the Customs and Border Protection (CBP) identify it. The form contains information about the goods carried on a means of transport and lists merchandise details and characteristics (such as quantity, value, and gross weight). It applies to brokers and carriers who transport merchandise in bond (an export or import shipment that has not yet been cleared by the customs) from a port of importation to another CBP port.

The main form - CBP Form 7512 - contains the information about the importer, the port of entry and unlading, the bond description, the reason for the transfer, and the record of the cartage or the lighterage along with the additional description of the goods. It is the main document that is filled out to sufficiently describe the cargo to identify it for the CBP.

The private parties may print the form if the printed version conforms to the official version in wording arrangement, size, color, and quality of the paper. All copies may be prepared simultaneously by carbon process unless more than one vehicle or vessel is used. In that case, prepare a separate manifest for each such vehicle or vessel.
The inspector has to fill out the report of the discharge at the destination indicating the port, the station, the actual date, the condition of the car, the seals, and the packages. The inspector's report includes the date of delivery to the importer or the general order, the description of the packages, the number and the kind of entry or the general order, the bonded truck or the lighter number, the conditions, the inspector's name, and signature.
