What Is Form MA1?
Form MA1, Maternity Allowance , is used when you will be expecting a baby but do not meet the qualifications for the Statutory Maternity Pay due to either your employment status or employment as an agency worker. This form works as an alternative way to qualify for Statutory Maternity Pay.

How to Fill Out Form MA1?
The Maternity Allowance Form requires the following information:
- Your full name and any previous names you have used, your birthdate, and your National Insurance Number.
- Your full address (and previous address if you have moved within the last three years), phone number, and email address.
- The projected date you are expecting to give birth or the baby's birthdate if they have already been born.
- Using the "Test Period" table, you will then calculate the average amount of money you received during a regular pay period to correctly estimate your maternity allowance. You can calculate the leave period by listing the starting time as the week before you expect your baby to arrive and counting sixty-six weeks after that time.
- You will write in both the projected start and end dates of your test period on Form MA1. There is also a series of yes-or-no questions regarding your employment status during the test period. If you answer yes to any of these questions you will be asked to complete additional questions later within the "Notes" section on Form MA1.
- If you will maintain employment during your test period, you will need to include information about your employer (such as name, address, phone number, dates of employment, and worker identification information). Provide information in regards to your normal pay periods as well as the date you last worked. Include any sick pay you may have received after going on leave, the date you plan to begin your leave if you have yet to take it, and what date you would like the maternity allowance to commence.
- Agency workers and self-employed workers will also be asked this information in the sections reserved for those individuals. Agency workers will be asked to include which agency they worked for. Self-employed workers will be asked for the date they registered as self-employed, and information about their self-employed business. If your spouse or civil partner is also self-employed you will need to provide their National Insurance Number, full name, and address. You will also be asked to include information about the marriage or civil partnership and if it has since been dissolved.
- If you were working abroad at any point during the Test Period you will need to include the general information and location of the company, and your employment status with the company.
- Include any additional benefits you are also currently receiving and the reference number.
- Provide the account number where you would like the benefit to be sent.
