DD Form 2715-2 Instructions
- Boxes 1 and 2 identify the prisoner by full name and registration number.
- Box 3 labels the form as an admission summary or progress summary. This choice will affect the fields that will need to be filed.
- Complete Line 5, Prior Offense, Line 6, Evaluation and Planning, and Line 7, Assignment Program, for admission summaries.
- Complete Line 7, Assignment Program, Line 8, Confinement progress, and Line 9.
DD 2715-2 Related Forms
The form is used for gathering information related to the prisoner and may become part of the clemency and parole-related paperwork package.
- DD Form 2715, Clemency/Parole Submission, and DD Form 2715-1, Disposition Board Recommendation, are submitted along with the DD 2715-2 part of the parole and clemency-related forms package given to the Clemency and Parole Board for review;
