DD Form 254 Instructions

The DD 254 contains 18 Items. Each Item has one or several boxes for filling.

1. The first group of items deals with levels of classification, facility security clearance, and safeguarding. Next items refer to the form itself, whether it is original, revised or final. In this section, the applicant also states if this form related to a follow-up contract or if this is the final version.
2. The next section requires the personal information of a contractor, subcontractor, and details on the location of the performance of their services. The applicant has to mark what kind of access the contract requires and what actions the contractor is allowed to do while performing their duties.
3. The form has to be dated and signed by either the contracting officer or authorized representative. The certifying official must also indicate their title and mailing address with a ZIP code. The last item of the form specifies where the form has to be distributed.

DD Form 254 has an unofficial continuation sheet distributed by some sources, that may be used if the provided space was not enough for all required information.

Where Do You Send a Completed Form DD 254?

It is advised to provide the form to DSS, contractor FSO, subcontractor FSO (if possible), the GCA POCs and all offices checked in the distribution box. Copies of the signed and certified form should be distributed to all addresses marked on the form.
