DD Form 254, Contract Security Classification Specification, is a form used to administer handling procedures for classified information, received or created on a basis of a classified contract. The form provides the contractor with security requirements and classification guidance required for completing a contract or agreement requiring access to the regulated material.
The latest edition of the form - sometimes incorrectly referred to as the DA Form 254 (Military Training Certificate - National Defense Cadet Corps) - was released by the Department of Defense (DoD) on April 1, 2018 . An up-to-date DD 254 fillable form is available for download below or can found on the Executive Services Directorate website.
Throughout the time of the contract, the DD Form 254 is reviewed every two years to assure that all the security requirements are adequate and relevant. This revision also secures the contract in case of changes in the security guidance.
This form is applied to all contracts that involve admission to classified material by U.S. contractors. The Federal Acquisition Regulation (FAR) requires this form for each classified contract.
The DD 254 contains 18 Items. Each Item has one or several boxes for filling.
DD Form 254 has an unofficial continuation sheet distributed by some sources, that may be used if the provided space was not enough for all required information.
It is advised to provide the form to DSS, contractor FSO, subcontractor FSO (if possible), the GCA POCs and all offices checked in the distribution box. Copies of the signed and certified form should be distributed to all addresses marked on the form.
