The Canada Immigration Family Information Form is supposed to be filled out by individuals who are applying for a Temporary Resident Visa or a Visitor Visa to Canada. The purpose of the application is to provide identifying information about the applicant's family members.

Completing the application is a straightforward process, the applicant only needs to make sure that all of the information that they provide in this document is truthful and accurate. According to the IMM 5645 instructions, all of the names in the application must be completed in both English and the applicant's native language (for example, Chinese, Korean, or Arabic). All of the family members must be included in the document, even those that are not accompanying the applicant. If an applicant needs more space on the form, then they should print additional pages with the sections they need and submit them with the rest of the application.

The IMM 5645 Form consists of three sections in total:

  1. Section A. This section is dedicated to information about the applicant, their spouse or common-law partner, their parents.
  2. Section B. All children, including all adopted and step-children, married children, children of applicant's spouse or common-law partner, and all of the applicant's children adopted by other people (regardless of their gender, age, or place of residence) must be indicated here.
  3. Section C. This section requires the applicant to list all of their siblings, including all half-sisters and half-brothers, step-brothers, and step-sisters.

Other information required on Form IMM 5645 includes the following:

  • The name and marital status;
  • Relationship to each listed family member ("mother," "father," "brother," "sister," "child," etc.);
  • Date of birth and country of birth of each family member;
  • If a family member is deceased, the applicant should indicate this and mention the city and the date the family member died;
  • Mailing address and present occupations of each family member (if a family member is unemployed, the applicant should state whether they are "studying," "retired," etc.)

An applicant must designate every family member mentioned in the form if they will accompany the applicant to Canada. If an applicant does not have a spouse, common-law partner, any natural, adopted, or step-children, then they must certify it on the form. IMM 5645 is a necessary part of the visa application. It is a supporting document that should accompany other documents mentioned in Form IMM 5484, Document Checklist for a Temporary Resident Visa. All of the forms must be filled out thoroughly and enclosed in the visa application package. Any information that the applicant will provide in their form will be collected and stored in Personal Information Bank in a Visitor Case File.
