NGB Form 34-1 instructions are as follows:
- Indicate the position you are applying for and the number of the job announcement. Provide your personal details - name, date of birth, current home address, and telephone numbers;
- State the date of your enlistment. Add your grade, military occupational specialty, specialty skill identifier, or Air Force Specialty Code, and the estimated time of separation;
- State the date of your federal recognition. Add your grade, branch, and the mandatory removal date. If you have a security clearance, provide information about it;
- Write down the details of your education and special qualifications. Name your college or university and state when you received your education. Indicate the degree program and credit hours. Specify your chief undergraduate and graduate subjects. If you have attended other schools or had other training, whether vocational, trade, or business, provide information on them. List your skills and qualifications along with any certificates or licenses you hold;
- Describe your employment history. Name your current and prior employers, add the dates of employment, the title of position, type of business, and the contact information of the employer. Try to describe your specific responsibilities and accomplishments in the provided space and state the reason for leaving your prior job;
- Provide information on your military history - service, training, and qualifications. Name your grades, organizations, and duties you performed, whether for the Active Component or the Reserve Component. Record the formal military schooling you have completed - the course title and number, duration of the course, and correspondence courses. List any military occupational specialties or specialty skill identifiers which have been awarded on orders. Indicate how you obtained these qualifications - on the job training, in the service school, from the civilian experience, etc. If certain qualifications have not yet been awarded, indicate their exact titles;
- Answer questions regarding your personal background. You are required to state whether you have been fired within the last five years or removed from military service. If you have even been convicted by a General Court Martial, check the appropriate box. If you have been under charges for any offense against the law during the past seven years, provide this information. Certify you will be able to complete a minimum of five years of continuous active guard/reserve service. If you need more space to fully explain any affirmative answers, use the continuation/remarks section. It is allowed to add separate sheets of paper if more space is required;
- Confirm all the information in the NGB 34-1 form is true and accurate. Sign and date the document.
