This is a supplemental document filled out by the minor's parents or legal guardians to authorize the issuance of a U.S. passport to the indicated minor if they cannot be present when the passport is executed.

Fill in this form when requesting a new or updated U.S. passport and it has not arrived in the mail within 10 days of it being sent out.

Use this form if you do not possess your original birth certificate or documentation that confirms naturalization, have no access to your birth records for any reason, or the birth records are insufficient, but you have to apply for a U.S. passport.

This form is requested in special situations when applying for a U.S. Passport for a minor under age 16
